Toxic Trough Infacnt/Peds: Q24>3 mcg/mL or Q8/Q12>10 mcg/mL; 18 - 150 yrs: less than 10 mcg/mL(reported from Cerner as less than/=9.9 mcg/mL for all ages 0-150 yrs.-ksk). Toxic Trough Adult: Q24>2 mcg/mL or Q8/Q12>10 mcg/mL
